Care Management Entity/Wraparound Evaluation

The Care Management Entity (CME)/Wraparound evaluation focuses on monitoring utilization, fidelity and outcomes for youth and families receiving wraparound services in the state of Maryland. In Maryland, we use a Care Management Entity (CME) structure to deliver wraparound. This service delivery model is being used currently for three federally funded grants: MD CARES, Rural CARES and CHIPRA, and was the service delivery model used for the CMS 1915(c) Medicaid RTC Waiver demonstration grant.

In July 2012, Maryland moved to a statewide CME contract funded by the Governor's Office for Children (GOC). Maryland Choices is that statewide CME provider and serves as an entry point for specific populations of children, youth and families with intensive needs so that they can achieve the goals of safety, permanency, and well-being through intensive care coordination using a Wraparound practice model and the development of home- and community-based services.

The CME is responsible for the collection of administrative data on the youth and families they serve. These data include how many youth and families were served; length of service; types of services received, reason for discharge from the CME; youth demographic characteristics; youth history of mental health and special education services; psychosocial functioning at entry into the CME, during and at discharge from the CME; and societal impact outcomes. In addition to administrative data, The Institute collects information from caregivers and youth at baseline, six months and 12 months after the start of services to monitor fidelity to the Wraparound model and resiliency/empowerment factors associated with service delivery. The Institute also plans to track longitudinal outcomes of youth served by the CME through state agency data systems. Semi-annual reports will be disseminated beginning in February 2013.
